At the end of the Family and Community Nursing module, the student:
identifies the person's care needs and their disease-related reactions; manages nursing care for chronic and disabled patients present in the area;
activates and supports the person's residual capabilities and promotes adaptation to the limitations and alterations produced by the disease, teaches risk factors and promotes a healthy lifestyle;
knows and promotes the patient's discharge in the area; explains to the user and the family the institutions present in the area and their usefulness;
activates and manages a helping relationship with the user, the family and other significant people by integrating with the other health and social figures present in the area;
develops educational projects with the person and the family to develop self-care skills;
educates the caregiver in managing problems and supports the patient and the family in the terminal phase of the disease.
